Care Coordinators

Our care coordinators work as part of a multidisciplinary team within our practice to identify patients in need of proactive support. This could mean, for example, patients living with frailty or patients with multiple long-term physical and mental health conditions.

They work across a number of fields, and in some cases may focus on a specific group of patients, such as a particular age group or those with particular health problems, for example they may work on re-calling diabetic patients, or contacting parents whose children are due their childhood immunisations, etc.
